Patient Frequently Asked Questions

Countryside Neurology is known locally for the advanced patient care provided by its doctor and staff.  We also provide a wide range of guest services and amenities to our patients. Learn more about preparing for a office visit, billing and financial services, and our other support programs.

  • Driver’s License or a valid ID
  • Insurance information
  • Referral letter (if required)
  • Reports, X-rays, MRI’s, CT scans etc. and any other relevant information
  • List of medications (if any)

We accept Medicare as well as most private insurances. Certain insurance companies do not necessarily contract with this office including all Medicaid plans; however, an important factor to realize is that medical insurance is a contract between the patient and the insurance company. This contract concerns the financial responsibility to pay for medical services provided by a doctor or other medical facility. Remember, it is the patient’s responsibility to confirm insurance coverage before you appointment, procedure or any other medical office visit.

During your initial visit, the doctor will take detailed medical history and perform neurological examination. He will explain the specific test or procedure you might need. Your therapy options will be discussed and plan of care will be established. Please arrive 15-30 minutes ahead of time in order to fill out necessary paperwork and HIPPA forms.

We ask that you give us a minimum of 24 hours or one business day notice if you need to cancel or reschedule your appointment. Otherwise, you will be assessed a $50 no-show fee, which is not covered by insurance. Patients with more than 2 no shows will be discharged from the clinic.

We accept checks, cash and all major credit cards including Visa, Master Card, Discover and AMEX. Your co-pay is required at a time of visit.

There are a few ways how you can get your routine medications refilled:

  • Call our office and request it. In case if there is no answer please leave a message with your name, date of birth, medication and its dosage, and pharmacy phone number. Please allow at least 1-2 business days to process your request
  • Register with our portal and request your refill through the portal
  • Ask your pharmacist to submit request electronically

Narcotics and most controlled substance medications refills require a follow up appointment. THERE ARE NO EXCEPTIONS!
