Process for new patients
- Patients with neurological issues interested in visiting Countryside Neurology can first call our office. If we accept your insurance, our staff will go ahead and help schedule your first appointment with us.

- Photo ID
- Insurance card(s)
- If you have insurance, you will be responsible for copays, co-insurance, deductibles, and any previous balance at the time of service

Your First Visit

- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent test results, x-rays, or relevant records
- Referral if coming from your primary Doctor (if applicable)
- Name of your pharmacy

Follow-up Visits
Any follow up visits will be made prior to your departure from our office. Our helpful staff will make every attempt to make sure your next appointment fits your personal schedule. In the event that you can not make your next appointment, please contact our office at least 1 business day prior to your appointment.
We will also help in making the necessary requests for appointments if you need additional special services like CT scans, EMG, EEG, or other additional tests that may be required.
